I agree, on my behalf and/or as the legal guardian/representative of the participant(s) under 18 years of age, that the participant is in suitable physical condition to engage in physical activity, or that they have been cleared by a physician to participate. I understand and agree that participation in this program is at the participant’s and my own risk and discretion. I consent to the use of photos and videos for advertising and social media purposes by Inner Warrior Fitness. I release Inner Warrior Fitness and its staff from any liability or responsibility for injuries or damages sustained during normal participation in these programs.

I acknowledge and agree to the cancelation policy in that I may cancel my membership at any time PRIOR to my payment date with no penalty. Once payment has been made, no refunds or credits are possible. I further acknowledge and agree that I may request to put a hold on my membership PRIOR to my payment date for up to 6 months, and that holds beyond 6 months are not possible. I must provide a specific beginning and end date for a hold to be processed.

If the $29.99 Intro Package Promo is being used, I understand and agree that this price is for the first month only and that I am signing up for an ongoing monthly membership. All future months will automatically be charged at the regular price for this program unless canceled BEFORE the billing date. Cancelations may be done by following the cancelation policy outlined above.
